Older patients being admitted to hospital are often prescribed medications that may no longer be safe and effective for them. Many older patients want their medications reviewed in hospital, but this often does not happen. This webinar will provide an overview of the deprescribing evidence base and describe the development and testing of the CHARMER hospital deprescribing intervention.
This webinar with be of particular benefit to pharmacists involved in providing or developing services for the care of older people, particularly those interested in overprescribing and deprescribing within the NHS.
The aim of this webinar is to provide an overview of the deprescribing research evidence base, draw attention to the barriers and facilitators of proactive deprescribing in the hospital setting and highlight the importance of incorporating this into routine practice. The webinar will present the work of the CHARMER team and the intervention they have developed to increase proactive deprescribing in UK hospitals.
After this webinar, attendees should be able to:
- Describe the current deprescribing evidence base with respect to impact on clinical outcomes and implementation in practice
- Describe the barriers and facilitators to deprescribing from the perspective of healthcare professionals
- Describe the development of a deprescribing intervention targeting geriatricians and pharmacists in hospital
